The Silicon Valley Chapter of the MOAA opens a new program to our membership, which is the Military ID Card Expiration Date Registry. It is a voluntary program in which you may provide us with your data, we will maintain your information in suspense and notify you of your impending ID card expiration date. This program is open to all active military and retiree cardholders, their spouses and other qualified cardholders.

The Chapter will receive your limited personal data, to include your military ID card expiration date. Chapter volunteers will notify you prior to your card expiration date and if that attempt fails, the listed alternate will be notified.

If requested, Chapter volunteers will provide you with the information to the nearest ID card issuing office. In addition, transportation can be coordinated and will be provided if available.

The bottom line is that your military ID card should not expire and its timely renewal is a must so that your military benefits are not interrupted!

If you elect to participate in our voluntary program, please fill out the form below and electronically submit it to our Chapter. A separate form must filled out for each cardholder with an expiration date.

Some guidance;

For Active Duty Personnel, your card will expire every three years, the date is normally found on the front side of your card.

For Retirees, the front side of your card will state "indef". Look on the back side to see if you have an "exp date". If so you must get a new card by that date.

For dependents, your card probably expires every four years. If you are a child, it may expire at age 21 or if you are a full time student, at age 23 or at the expected date of graduation. The date is on the front, right top of the card.

When filling out the form below, use the proper format in submitting dates to eliminate confusion (mm/dd/yyyy).

When you receive a new card, please send us the change data if you have a new expiration date!
